The office is on the 3rd floor of the historically registered Bank of the West building, located on the northeast corner of East Colorado Boulevard and Madison Street, a few blocks east of Los Robles and about five blocks west of Lake Avenue.

Visitor parking is available behind the Bank of the West building on Madison. Sorry, we do not validate parking. There is one hour free parking on the local streets and other pay-to-park lots are also available in the immediate area. There are restaurants and hotels in the immediate area for patients coming from out of town for assessments.
